Warranty Submissions

Use this form to submit an Epix Hockey stick warranty claim. Please complete every required field and include clear photos so we can properly review the claim.

Before submitting, please have:
  • Your order number or valid proof of purchase.
  • The stick model/specifications and serial number, if present.
  • A clear explanation of how the damage occurred.
  • Six clear photos showing the complete stick, the damaged area from multiple angles, and the serial number.

Please keep the damaged stick until the claim has been fully reviewed.

Upload six clear images. JPG, JPEG or PNG recommended. Keep the combined upload size under 10 MB.

Submitting a claim does not guarantee warranty approval. Epix Hockey will review each claim to determine whether the failure is consistent with a manufacturing defect.
